JMA Health and Wellness offers commercial drivers the physical exam they need to be compliant with their CDL license. The DOT physical is required to be performed by a certified FMCSA Medical Examiner.

- List of medication/supplements names and dosage
- Corrective Lenses/Hearing Aids
- Diabetes patients need to bring copy of lab report with current A1C
- Compliance report for Sleep Apnea
- Medical Clearance letter - if you have any medical conditions involving Heart, Mental Health, Chronic Pain, etc
- PLEASE REMEMBER THAT WE WILL COLLECT A URINE SAMPLE WHEN YOU ARRIVE
